The company is NOT in violation of State law by not reporting the accident. Only in cases of fatalities or in special cases such as mechanical power presses is it necessary to to notify IOSHA

Under the revised OSHA/IOSHA Fatality Catastrophy rule, If three people are admitted to the hospital or there is a death due to an accident,  the accident must be reported to IOSHA with in 8 hours.
