ELKHART - Terry Lee Ellsworth, 57, of Middlebury, passed away at 10:35 p.m. Friday, Oct. 12, 2012 at Hospice House of Elkhart.
He was born Jan. 27, 1955, in South Bend to Robert Lee and Joyce (Gundrum) Ellsworth.
On May 23, 1987, in Elkhart, he married Debra E. Quinn.
She survives along with two sons, Robert Lee II (Nicole Thurston) Ellsworth of Middlebury and Terry Lee Ellsworth Jr. of Buchanan, Mich.; two daughters, Heather Marie Scott of South Bend and Lisa Dawn (James) Dobkins of Goshen; a stepson, Willard J. Quinn of Elkhart; and two stepdaughters, Tara Quinn-Freeze of Bristol and Stacy (Charles) Gonyon of Elkhart. Also surviving are 16 grandchildren; one great-granddaughter; three sisters, Jackie Elam of Edwardsburg, Mich. and Linda Searfoss and Roxanna (David) Keiling, both of Elkhart; and a brother, Jerry K. (Cherrie) Smith of Elkhart.
Terry was preceded in death by his parents, Robert Lee and Joyce Ellsworth.
Terry was a former cabinet maker and forklift driver for the Mobile Home and R.V. Industry.
He loved fishing, drawing, woodworking and being outdoors.
Visitation for Terry will be from 5 to 7 p.m. Friday, Oct. 19, 2012, with a funeral service at 7 p.m. at Walley-Mills-Zimmerman Funeral Home.
Cremation will follow.
Memorial contributions may be given to the Hospice House of Elkhart.
